Hours 0 to 24: The Critical First Window
The first 24 hours after stem cell injection are the most important for protecting the treatment. NSAIDs (ibuprofen, naproxen, aspirin, Aleve) must be avoided. NSAIDs suppress the inflammatory signaling cascade that the injected MSCs are using to communicate with the surrounding tissue. Taking anti-inflammatories in this window directly counteracts the mechanism of action.
Ice at the injection site is also avoided in the first 24 to 48 hours. Cold reduces local blood flow and metabolic activity at the injection site, slowing the biological processes the treatment is initiating.
Most patients experience mild soreness at the injection site that resolves within 24 to 48 hours. This is expected and does not require intervention beyond rest.
Hours 24 to 72: Light Activity Acceptable
By 24 to 48 hours post-injection, most patients resume light daily activities. Walking is encouraged. Sitting for extended periods is fine. The restriction is on heavy loading of the treated area, high-impact activity, and strenuous exercise.
For IV protocol patients (systemic stem cell therapy), restrictions are lighter: mild fatigue is the most common day-1 experience, and most patients resume normal activity within 24 hours.
The Recovery Protocol: Weeks 1 to 4
NSAIDs remain off-limits for at least 7 to 10 days post-injection. Corticosteroid injections at the treated site should be avoided for 4 to 6 weeks, as they suppress the very inflammatory signaling the treatment is leveraging.
Progressive return to full activity is guided by symptom response. A physical therapy or home exercise protocol is often recommended starting at week 2 to support tissue remodeling during the repair window.
