PRP or Exosome Therapy — Which Is Better?
Neither is universally better. PRP uses the patient's own concentrated platelets and is often the appropriate first biologic for earlier-stage problems, while exosome therapy delivers a donor-derived extracellular vesicle signal that is more concentrated and consistent and is typically considered for more advanced or PRP-unresponsive presentations.
The Core Difference
PRP is autologous. The material comes from your own blood, drawn and processed in a single visit. That eliminates rejection risk, allergic reaction to foreign cells, and donor-derived pathogen transmission — but it also means the quality of the product depends on your own platelet count and general health, which varies between patients and with age.
Exosome therapy is allogeneic. The vesicles are donor-derived from umbilical cord tissue and processed to a controlled specification. The signal is more concentrated and more consistent between patients, and it does not depend on the recipient's own biology.
Where Each Tends to Fit
PRP is often the sensible first step for earlier-stage joint degeneration and chronic tendinopathy, particularly in younger and metabolically healthy patients whose own platelet quality is good. It is also the lower-cost entry point into biologic treatment.
Exosomes are typically considered where degeneration is more advanced, where PRP has been given a fair trial without adequate response, or where a patient's own biology limits what PRP can deliver.
They Are Not Mutually Exclusive
Both are sometimes used within a combined biologic protocol rather than as competing options. Which applies depends on the diagnosis, the stage, imaging, and your overall health — that is a clinical decision made at assessment, not a preference selected from a menu.
