Why Are Doctors Hesitant to Prescribe Testosterone?
Doctors are cautious about testosterone because it suppresses the body's own production and fertility, raises red blood cell mass, requires ongoing prostate and blood monitoring, and is frequently requested for symptoms caused by something else entirely, such as sleep apnea or thyroid disease.
The Four Clinical Reasons
1. It suppresses your own production. Exogenous testosterone signals the body to stop producing its own, and it can significantly impair sperm production — in some cases persistently. For a man who may still want children, this is the single most important consideration, and it is discussed before therapy rather than after.
2. It raises hematocrit. Testosterone increases red cell mass, which thickens the blood. This is silent without bloodwork and is the most common reason a well-running protocol needs a dose reduction.
3. Prostate monitoring is required. A PSA baseline must be established before therapy and re-checked on schedule.
4. The symptoms usually have a differential. Fatigue, low drive, poor recovery, and cognitive fog are produced just as readily by untreated sleep apnea, thyroid disease, anaemia, depression, medication effects, and chronic sleep debt. Treating those with testosterone masks them.
