What Are Muscle Adhesions? Why a Knot Is Not Just Tight Muscle
Adhesion, Spasm, and Tightness Are Three Different Things
These terms get used interchangeably, and the distinction determines what treatment will actually work.
Tightness is a muscle held at shortened length, typically postural. It responds to stretching and to changing the posture that caused it.
Spasm is an involuntary sustained contraction, usually protective and often acute. It responds to rest, heat, and time.
Adhesion is a structural change. The fascia — the connective tissue sheath wrapping every muscle and separating muscle layers — has thickened and bound to adjacent tissue. Layers that should glide against one another no longer do. This does not respond to stretching, because you are not stretching a shortened muscle; you are pulling against tissue that has physically bound together.
How Adhesions Form
Adhesions develop where tissue is subjected to sustained low-grade load, repetitive motion, or injury followed by immobility. The sequence generally runs: sustained contraction reduces local circulation, reduced circulation drives an inflammatory and fibrotic response, and fibrotic tissue lays down disorganized collagen that binds structures together.
Collagen laid down under these conditions is not aligned along normal lines of force the way healthy tissue is. It is functionally scar tissue inside otherwise healthy muscle, and it is both less elastic and less well perfused than the tissue around it.
Why Stretching and Foam Rolling Fall Short
Stretching lengthens muscle fibers. It does not remodel fibrotic collagen. Foam rolling and manual pressure can produce a temporary reduction in muscle tone and a genuine sense of release, but the mechanical force applied through a roller or a thumb is generally insufficient and too diffuse to remodel an established adhesion.
This is the source of the pattern nearly every patient describes: real relief that lasts hours or a day, then a return to baseline.
What Does Affect Adhered Tissue
Remodeling fibrotic tissue requires a stimulus strong enough to disrupt the disorganized collagen and trigger a repair response that lays down better-organized tissue in its place. Radial shockwave therapy applies acoustic pressure waves for exactly this purpose — the mechanical stimulus initiates a controlled healing response including increased local blood flow and collagen turnover.
Therapeutic ultrasound serves a complementary role. Deep sound-wave heating raises tissue temperature, which increases extensibility and reduces muscle tone, making the tissue more responsive and helping prevent the muscle from re-tightening between shockwave applications. Note that this is therapeutic ultrasound as a treatment modality, not the diagnostic ultrasound used elsewhere in the clinic to guide injections.
Timeframes Are Realistic, Not Instant
Because the mechanism is tissue remodeling rather than symptom relief, meaningful change takes weeks rather than a single visit. Shockwave applications are spaced a week apart deliberately: the tissue needs recovery time between applications for the remodeling response to proceed. A course typically runs six weeks.
Patients whose expectation is set around a single dramatic session are frequently disappointed by conservative care of any kind. Patients who understand they are remodeling tissue over a couple of months tend to report a very different experience.
