Does TRT Cause Testicular Shrinkage?

Yes, testicular atrophy is a recognised effect of testosterone replacement therapy. Exogenous testosterone suppresses the pituitary signals that stimulate the testes, and reduced stimulation leads to reduced testicular volume over time. The degree varies between individuals and is often partially reversible after stopping.

Why It Happens

Testosterone production is controlled by a feedback loop. The pituitary releases luteinising hormone (LH) and follicle-stimulating hormone (FSH), which instruct the testes to produce testosterone and sperm. When testosterone is supplied from outside the body, that feedback loop reads levels as sufficient and reduces LH and FSH output. The testes receive less stimulation, and tissue that is not being stimulated reduces in volume.

The same mechanism explains why testosterone therapy suppresses fertility — the drop in FSH reduces sperm production alongside the change in testicular size.

How Much and How Reversible

The degree varies considerably between individuals and with duration of therapy. It is often partially or substantially reversible after discontinuation, though recovery time varies and is not guaranteed to be complete.
